Office Policies:
The patient is expected to bring some form of payment to the first visit.

Emergencies:
– All Emergencies will be fit into the doctor's schedule the day of the emergency.
– For after hour emergencies, the doctor can be reached by calling his mobile number, which can be found on the answering machine message.

Method of Payment:
– We accept all major credit cards including Visa, Mastercard, Discovery and Amex.
– For patients with no insurance who cannot afford the treatment up front, we can assist in getting credit through Care Credit (Third Party Payment Company), which allows them to pay for their treatment over time.

Insurance Policy:
– The patient is expected to pay 1/3 of the total cost of treatment before the end of treatment. At that time, the insurance will be submitted electronically. When payment is received by our office, either a check or a bill will be sent to the patient, depending whether that payment has satisfied the existing balance or if there is an overpayment.
– Before treatment is started, the patients insurance company will be called to provide each patient with the amount left in the insurance for that year and the percentage amount the insurance company will pay for the patient's specific treatment.

Note: It is the patient's responsibility to bring all information, including primary and secondary insurance cards, on the first visit. We accept most major insurance plans.
